Enter your ATHLETE’S full name

Sign and Date

Enter your (parent/guardian) name

Sign and Date

Submit and wait until you see “You’ve successfully signed the document!”

This is a REQUIRED form that needs to be filled out and signed when you sign up for any class. Please make sure to fill this out and submit.
